01Duke in Forms

Describe the form. Duke builds it.

Say what the form is for and Duke proposes the questions, where each answer saves and what happens after. Nothing is made or published until you approve it.

Who decides

  • Waits for your approvalHe prepares it and shows a one-line preview. Nothing changes until you approve.
  • Just answersReading needs no approval, and he sees only what the person asking may see.
Meet Duke
01Waits for your approval

A form from one sentence.

Duke drafts the questions and the after-sending steps, and makes the form once you say yes.

Ask about the answers.

Duke reads the answers your role may see and tells you what came in.

How many people sent the demo class form this week?

Ideas for your kind of business.

New forms start from ideas made for your business type, such as a service request, an admission enquiry or a trial booking, or from a blank form.

How it works.

What Forms does, part by part, on screens from our live demos.

01Build

The questions you need, saved where they belong.

Add a text box, a longer answer, a dropdown, multiple choice, a date, a number, a file upload or a consent tick. Each answer says where it saves: the contact's name, phone or email, one of your extra fields, or the lead. A question can show only when an earlier answer matches.

  • File uploads up to 5 MB, checked for viruses first
  • Consent keeps the exact words and the time it was ticked
  • Hidden fields record which poster or ad someone came from

02After sending

Every answer turns into work you can see.

When someone sends the form, the contact is found by phone, then email, or made new, so one family never becomes two. Choose whether it starts a lead in a pipeline stage, who is told, and whether visitors see a thank-you message or go to another page.

  • Fill in only what's empty, or use the newest answers
  • A lead in your chosen pipeline and stage, or a note on their open lead
  • Automations can start with “A form is sent”

With Duke: a new answer lands on the contact's timeline, so he knows about it when you ask.

03Share

One form, everywhere you meet people.

Once it's published, a form gets its own link for WhatsApp, Instagram or SMS, a QR code and a printable A4 poster, a section for any page of your website, and a code to paste into WordPress, Wix or any other site. Give each poster or post its own link ending to see where answers came from.

  • Your brand on the form, on any of your addresses
  • Switch off “Allow on other websites” to keep it on yours
  • Close it on a date or after a number of answers

06Questions

Forms: common questions

Can I put a form on my existing website?

Yes. Copy the form's code and paste it into WordPress, Wix, Squarespace or any page's HTML. It shows in a frame with the height you set, and every answer lands in DukeOS.

Do answers create contacts and leads?

Yes. Each answer finds the contact by phone, then email, or makes a new one. You choose whether it also starts a lead in one of your pipelines, and who on the team is told.

How do you stop spam?

Forms have a hidden trap field, a minimum time to fill in and limits on how many answers one address can send. Real visitors never see a puzzle.

Can people pay inside a form?

Not yet. A form collects details, files and consent. To take a payment, link to booking or your shop on your website, or send an invoice.
