Forms: build, share and follow up every answer

Build a form, choose where each answer saves and what happens after, share it as a link, QR poster, website section or code for another site, and see the answers.

On this page

A form collects details from people outside your business: an enquiry, a trial booking, an admission, feedback or a service request. Every answer finds or makes the contact, can start a lead and tells the right person. Open Forms in the menu (under Messages & Marketing). If you don't see it, switch the Forms app on in Settings.

Make a form

  1. Open Forms. Under What would you like people to fill in? pick one of the ideas for your type of business, or Describe your own form to tell Duke what it's for.
  2. Or click New form, give it a Form name and add the questions yourself.

Add questions

On Build, add questions from the list on the left. Drag them to reorder, and click one to change it.

  • Contact details: name, phone, email and address.
  • Questions: text field, longer answer, dropdown, multiple choice, date, number, file upload, consent tick and hidden field.
  • Your extra fields: custom fields already on your contacts, so answers fill them in directly.

For each question:

  • Saves to: where the answer goes, such as the contact's name or phone, one of your extra fields, or the lead's title, value or source. Or keep it only with the answer. You can make a new extra field from a question.
  • Required: the form can't be sent without it.
  • Show this question: show it only when an earlier dropdown or multiple-choice answer matches.

Good to know:

  • A file upload takes one file up to 5 MB (photos, PDF or Office documents). It's checked for viruses before it's kept, and only your team can open it.
  • A consent tick keeps the exact words shown and the time it was ticked.
  • A hidden field keeps something visitors don't type, such as which poster or ad they came from.

Set the Heading, A line under it and the Button text at the top. The form uses your brand from Settings → Brand.

Choose what happens

On Settings, the steps run in order when someone sends the form:

  1. Find or add the contact: we look for the same phone number first, then the same email, so one person never becomes two. Choose Fill in only what's empty or Use the newest answers, and add a tag if you like.
  2. Start a lead: in a pipeline and stage you choose, given to one person or in turn. If they already have an open lead in that pipeline, the answer is added to it instead.
  3. Tell your team: the people you pick get it in the bell (and by email if they've chosen that).
  4. Run an automation: any automation that starts with A form is sent can pick this form. See Automations.
  5. Then show: A thank-you message or Another page (a web address).
  6. Keep it open: close the form on a date or after a number of answers, and choose what people see when it's closed.

Publish and share

Click Publish. Later changes wait until you click Publish changes. Then open Share:

  • Its own link: for WhatsApp, Instagram, posters and SMS. Copy it, Download QR (PNG) or Print an A4 poster. Give each poster or post its own Link ending to see where answers came from.
  • On your website: pick a page and the place, and a Form section is added to your website's draft. Publish your website to show it. Later changes to the form show on the site at once. See Your website.
  • On another website: copy the code and paste it into WordPress, Wix, Squarespace or your own site. Set the Height in pixels. Turn off Allow on other websites to keep the form on your own addresses.

Answers

Open Answers to see views, answers and the finish rate, a chart for each question and a table of every answer. Filter by date and by where people came from. Open an answer to see what happened (the contact, the lead and who was told), with links to the contact and lead. Download the answers as a CSV file.

Who can build, share, read, download and delete answers depends on their role. In the demo businesses, downloading and deleting answers are switched off.

Spam

There's no puzzle for visitors. Forms have a hidden trap field and a minimum time to fill in, and limit how many answers one address can send in an hour. Spam is dropped quietly.

Not yet

Forms are one page each, the code for other websites has a fixed height, and people can't pay inside a form. To take payment, use booking or the shop on your website, or send an invoice.
